Community Service Worker – Per Diem 25 hours per week Division of Senior Services, Department of Human Services

Organizational Overview:

The Bergen County Division of Senior Services is the primary agency in Bergen County that plans and coordinates services for county residents aged 60 and above, and their caregivers. The Division and its various committees advocate for those with the greatest economic and social needs.

Job Description:           

This employment opportunity is to serve as a Community Service Worker.  Under supervision, performs a variety of routine field and office work involved in community-oriented programs such as housing, urban renewal, education, welfare, unemployment, and crime prevention; interviews community residents and assists them in obtaining government aid and support services, or makes referrals to other agencies; does other related duties as required.

Job Responsibilities:              

  • Create and implement individualized Plans of Care. Meets with clients in home and assess needs, living arrangements, strengths, and support networks to determine their goals
  • Develop care plans to improve well-being, and arrange formal and informal supports
  • Provides referrals to other agencies for additional services.
  • Provides assistance to residents within the scope of agency functions.
  • Make quarterly visits and monthly calls to determine whether original goals are being met. If necessary, revise the service plan to accommodate changing needs.
  • Coordinate with government agencies to complete benefit applications for the client
  • Assist clients and their families plan for future needs as health of client changes.
  • Prepares reports of program activities in strict compliance with policy, regulation, and procedure.
  • Attend meetings and trainings as required.
  • Stay well-informed of community resources and procedures for referring clients to appropriate resources.

Schedule:   Per Diem Up 25 hours

Education Requirements: 

  • Licensed or Certified Social Worker, or Registered Nurse, or Bachelor’s Degree in a health-related or behavior science field.

Other Requirements:

  • One (1) year of experience working with older adults or people with disabilities, in a institutional or community setting.
  • Will be required to learn to utilize various types of electronic and/or manual recording and information systems used by the agency, office, or related units.
  • Proficiency in Microsoft Office Suite, including Teams, Word, and Excel programs.
  • Bilingual English – Spanish is preferred
  • Valid New Jersey Driver’s License
