Bergen County Department of Health Services and Local Hospitals Seek Community Input in 2025 Health Survey
The Bergen County Department of Health Services and the Community Health Improvement Partnership (CHIP), in collaboration with Bergen County’s seven non-profit hospitals, have officially launched the 2025 Community Health Needs Assessment (CHNA) survey.
